Tackle Football Equipment Distribution Makeup Date is
Monday, August 19, 2019 from 11:00 AM-1:00 PM @  Middlebrook (meet at the Cube Smart Storage unit outside by the football field. Look for the WYF sign)
 

TIME SCHEDULE                                    
Distribution arrival times are set by grade, please try to attend during your assigned time! 

  • 11:00 - 12:00pm     grades 3-5
  • 12:00 - 1:00 pm      grades 6-8

PLEASE make every effort to attend the equipment pickup on 8/19. For those who do not pick up equipment on 8/19, you will be assessed a $25 special handling fee for equipment pickup later in the week.

In order for you to receive your equipment, please note that YOU MUST:

  • Be Present:  To ensure equipment is fitted properly and the for the safety of our players, equipment will not be distributed unless the player is present.
  • Be registered:  Completed all registration forms online.
  • Be paid up:  Registration fees must have been paid in full.
  • Bring the following with you:
    • Your completed Medical Form (ONLY the WYF Medical form is accepted)
    • Your completed Parental Consent and Waiver of Liability Form
    • Emergency Form for players with severe limitations (basic form is completed online)
    • An equipment deposit check in the amount of $400, payable to "WYF", and DATED 12/1/19

*Equipment Note: If you miss the 8/19 pickup, you will need to notify the coach on the first day of attendance to make arrangements to get your equipment and there will be a $25 extra charge.
